I think I saw it more as Eiji being bowled over by how deep Oishi thinks about this kind of stuff? And then, thinking about himself in that same sense ("If I could do a job that embodied skills I possess to make others happy, what would it be?"), ponders stunt work.
I do love how it closed with Oishi assuring Eiji--and himself--that even though this is important to Oishi, he still wants them to be together in any way possible ;__;
I did wonder if it was foregoing being an actual doctor, or if that would be his normal job and being Eiji's trainer would be his fun job? :D
Or maybe he plans on being a Physical Therapist? I can totally see that, and as often as Eiji would then be getting into scrapes and sticky situations (*coughcough*) it'd be perfect to keep them connected :D
